Having managed industrial maintenance programs for 40 years, I can say with certainty that "unusual events" are very usual, in a scenario of moving parts, energy transfer, and the real contact of real people with real events.

No machine ever devised was perfect. That said, the nurturing of industrophobia helps no one. The perfection of future utopias is comforting to those spending months & years in their contemplation. However, future utopias have never fed, housed, clothed, or enlightened a single human soul, and that is their downfall.

But a job is a job, and your job is to make citizens afraid of their own blessings & advantages. Nice work, people.
